Beeson Project- Medication Optimization for Older Adults with Dementia
This study aims to improve medication safety for older adults with Alzheimer’s disease and related dementias during the transition from hospital care to home health care. Many individuals with dementia are prescribed medications, such as antipsychotics, that may no longer be appropriate, yet reducing these medications safely can be challenging.
Researchers are engaging patients, caregivers, clinicians, and home health professionals to identify barriers and supports to appropriate medication reduction. Their input will be used to refine HomeMed‑APD, a home‑based deprescribing program designed to support safer, patient centered medication uses in home health care.
