Many individuals with dementia are prescribed medications, such as antipsychotics, that may no longer be appropriate, yet reducing these medications safely can be challenging.<\/p>\n<p>Researchers are engaging patients, caregivers, clinicians, and home health professionals to identify barriers and supports to appropriate medication reduction. Their input will be used to refine HomeMed\u2011APD, a home\u2011based deprescribing program designed to support safer, patient centered medication uses in home health care.[\/vc_column_text][vc_separator border_width=&#8221;1px&#8221;][vc_column_text]<\/p>\n<h4>Interested in getting involved in the HomeMed-APD Study?<\/h4>\n<p>See below for more information on each of the opportunities available within the program.
